Member Re-Test Forms

We are so excited you have decided to retest and continue your journey with ITA! We are so excited to have you! All the forms you need to fill out for your re-assessment are below.

 

You can also find the forms you need if you are looking to add an additional animal to your membership!

You will also be able to submit your animal health screening, fecal test results, and rabies vaccination evidence in PDF form below.

Please print this questionnaire and bring it to your scheduled team assessment. You need one of these for each animal you test with. You can download the form below.

Please print this out, have your vet fill it out, and then submit a PDF scan via the "Completed Animal Health Screening" section, email it to us, or turn it in to the office. You need one of these for each animal you test with.

If you did not have your completed Animal Health Screening, Fecal Test Results, or Proof of Rabies Vaccination when you completed your re-test application, they can be submitted using the links below.
