You will also be able to submit your animal health screening, fecal test results, and rabies vaccination evidence in PDF form below.

Electronic Submissions

Please use the link below to fill out the membership application form. Each person in your family that has tested to be a handler needs to fill out a form. You will submit your Animal Health Screening and Fecal Test Results through this form.
