New Member Forms

Congratulations on becoming a member of Intermountain Therapy Animals! We are so excited to have you! All of the forms you need to fill out can be found below.

You will also be able to submit your animal health screening, fecal test results, and rabies vaccination evidence in PDF form below.

Please print this questionnaire and bring it to your scheduled team assessment. You can download the form below.

Please print this out, have your vet fill it out, and then submit a PDF scan via the "Completed Animal Health Screening" section, email it to us, or turn it in to the office.
